Screen Replacement Program Terms and Conditions | |

1. | Screen Replacement Program(the “Service”) is provided to you by Bolttech Device Protection Hong Kong Limited (“bolttech”). You confirm to have read, understood and accepted the terms and conditions of bolttech (the “Screen Replacement Program Terms and Conditions”) as stated in https://restart-device.bolttech.hk/hk/en/terms-and-conditions. |
2. | Application for the Service is subject to approval by bolttech. |
3. | Unless otherwise explicitly indicated by us in writing, the Service is available only to HKID or passport holders who have subscribed to our post-paid services. |
4. | The Service is only applicable to eligible device models (“Eligible Device”). |
5. | To apply for the Service, you must subscribe to designated mobile plans (“Designated Mobile Plans”) and designated roaming value-added service plans at the time of Service subscription; or you have an Eligible Device, please refer to https://restart-device.bolttech.hk/hk/en/device-plans for details. |
6. | Eligible Device: eligible device model which are purchased from csl, 1O1O as a standalone device or a designated retailer; and the device owned by you. |
7. | Eligible Device must be in good conditions. Upon your application, you will be required to present your device for inspection to see if it is eligible for the Service, as well as provide the device model and IMEI number of your device. |
8. | The Service allows you to replace a screen for your registered device (“Screen Replacement”) during the Service period by paying to bolttech a replacement fee (the “Screen Replacement Fee”) of 3% of the Original SRP. The above service is collectively called “Service Requests”. You will also be required to pay a monthly subscription fee (HK$20/month) for the Service. Screen Replacement Program Terms and Conditions apply. For full terms of the Service, please see Screen Replacement Program Terms and Conditions. |
9. | Screen Replacement Service is only applicable to smartphone or tablet. If you submit a Screen Replacement request, our Service Provider will replace your Registered Device screen. For full terms and conditions of the Screen Replacement Service, please refer to the Screen Replacement Program Terms and Conditions linked in paragraph 1 above. |
10. | If customer fails to provide IMEI number of the Eligible Device when applying this service, bolttech system will arrange a set of temporary IMEI numbers to bolttech until customer can provide it. You must provide your Eligible Device’s IMEI number to bolttech in order to be eligible for any Service Request. If you do not provide such IMEI number at the time of application, please ensure that it is provided to bolttech subsequently. Service Request will only be accepted if the IMEI numbers of the device you present for such Service Request matches with the IMEI number of the device you registered as the registered device. The paid Service fees are not refundable. Signed contract period will not be changed and extended, fees will not be refunded. |
11. | Please refer to Screen Replacement Program Terms and Conditions for the number of screen replacement you are entitled to during each 12-month rolling period during the Service period. Screen replacement service is available to Eligible Device only. |
12. | The Service, including the delivery of your Registered Device back to you after screen replacement, is provided to you by bolttech. CSL Mobile Limited (“CSL”) expressly disclaims any warranties or representations of any kind in relation to the Service (whether express or implied). |
13. | CSL is not the supplier of the Service, and makes no representation or guarantee as to the quality of the Service provided by bolttech. In case of any enquiry or dispute about the Service, please contact bolttech directly on 58032496. |
14. | If you terminate the Service before the expiry of the commitment period for the Service, CSL will charge you early termination charges, which are the sum of the monthly fees for the Service for the remaining unexpired period. |
15. | The Service will terminate immediately when your Designated Mobile Plan is terminated by you or by CSL. |
16. | The General Terms and Conditions of CSL apply (https://www.hkcsl.com/en/service-terms-and-conditions/) to the relationship between CSL and you in respect of the Designated Mobile Plan and the Service. |
17. | In the event of any discrepancies between the Chinese and English versions of these terms and conditions, the English version shall prevail. |

NETVIGATOR Extra Service – Screen Replacement Program Terms and Conditions: | |
1. | Screen Replacement Program (the “Service”) is an extra service under HKT NETVIGATOR Broadband Services (provided by Hong Kong Telecommunications (HKT) Limited (“HKT”)). The service is applicable to all NETVIGATOR customers. Eligible customers need to subscribe Screen Replacement Program with at least a 24-month commitment period (the “Restart commitment period”). During the Restart commitment, you need to use NETVIGATOR broadband service. |
2. | The Service is provided by Bolttech Device Protection Hong Kong Limited (“bolttech”). You confirm to have read, understood and accepted the terms and conditions of bolttech (the “Restart Program Terms and Conditions”) as stated in https://restart-device.bolttech.hk/hk/en/terms-and-conditions . |
3. | After The Service takes effect, the redemption code will be sent to the customer's registered mobile number via SMS within 7 working days by HKT. It includes the redemption code, how to register on bolttech platform and how to use the redemption code. Redemption code cannot be returned, refunded, resold, transferred, and exchanged for cash and/or other gifts. |
4. | The Service fees will be added to your NETVIGATOR Broadband Services bill |

12. | If you do not agree to continue subscribe the service after the commitment period for the Service, please submit your termination request at least 30 days prior to your requested service-end. You can call our customer service hotline on 1000 or visit to csl/1O1O shops to get a termination request form. |
13. | If you terminate the Service before the expiry of the commitment period for the Service, we will charge you early termination charges, which are the sum of the monthly fees for the Service for the remaining unexpired period. The Service will terminate immediately when your NETVIGATOR Service is terminated by you. |
14. | At the end of the Restart commitment period, if (a) HKT has not received any valid written termination request from you regarding the Screen Replacement Program; or (b) you have not renewed your existing contract and have not entered into any new contract with a new commitment period for the Screen Replacement Program, HKT will continue to provide the Screen Replacement Program to you as an extra service to the NETVIGATOR broadband service on a monthly basis in accordance with the existing contract, until you provide HKT with at least 30 days' advance written notice to terminate the Screen Replacement Program. |
15. | HKT is not the supplier of the Service, and makes no representation or guarantee as to the quality of the Service provided by bolttech. Neither do they accept liability for any matters arising from, or in relation to, the same. Any dispute or complaint in connection with the Service will be referred to bolttech. In case of any enquiry or dispute about the Service, please contact bolttech directly on 58032496. |
16. | HKT reserves the right to change or terminate the service and/or these Terms & Conditions at any time without notice. In the event of dispute, decisions made by us shall be final and binding. |
